The Educator submitting this form is required to list an accurate legal guardian for this student.
The Oakland Public Education Fund takes no responsibility to identify guardianship.
Guardians are required to have email addresses.
Student must have access to a devise and the internet. If this is an issue, please connect with TechExchange.
